Premier Health Group Network (PHG): PHG is a member of the Midwest Health Collaborative. As a Premier Health employee, when you or your covered family member obtain care from a PHG provider, you will generally receive a higher level of benefit. PHG includes Premier Health facilities, Dayton Children's Hospital, Cincinnati Children's Medical Center and Wayne Healthcare. CVS Clinics nationwide.

Your Extended Provider Network - In the State of Ohio

Within the state of Ohio, you may obtain care from other Midwest Health Collabortive (MHC) network providers (Aultman, Cleveland Clinic, Ohio Health, ProMedica, TriHealth). When you obtain care from a MHC network provider, you will generally have a higher out of pocket expense.

Outside the state of Ohio, you may obtain care from a PHCS network provider. When you obtain care from a PHCS network provider, you will generally have a higher out of pocket expense.
